media upload form not working in wp settings

The goals here is to make an upload button in the settings wordpress page, the problem here is the “select file” button didn’t pop up the media gallery window that needed for me to get the image / upload the image

this prompt didn’t help because it just redirect to default media and again select file button is still not working

i have searched something with this, but most of them are not specifically helping this case… is there anything i need to change in the code?
